We will work diligently with Patients, Family members, Case Managers, Social Workers and Physicians to transition patients from the hospital to their home setting safely. Our Nurses will act as an Ambassador to the Physician to ensure the patient is properly following discharge orders. We will use care path protocols for management of specified diagnosis and change in condition forms, as well as update the Patient’s Physician weekly. We provide Intermittent, Acute skilled nursing services and multiple therapies to patients with chronic illnesses, recent injuries and Patient’s recovering from surgery.

Home health is a medical benefit available to qualified patients who require skilled nursing, rehabilitation, bath aide, and other services at home. Crescent Home Healthcare is a Medicare Certified Agency who has earned the Medicare Gold Seal. The Joint Commission also accredits us. In most cases, Medicare will pay for all the services. ZERO out-of-pocket dollars.
