General Programs

Crescent Home Healthcare, Inc.’s primary objective is to deliver high-quality medical care directly to patients in the comfort of their own homes. We will collaborate with patients, family members, case managers, social workers, and physicians to safely transition patients from the hospital to their home setting.

Skilled Nursing

Care by our exceptional Registered Nurse includes, but not limited to:

  • Comprehensive head-to-toe assessment each visit
  • Need assessment including safety, DME, therapy, social worker, labs etc.
  • Disease management, instructions, and teachings
  • Medication reconciliation and management
  • Diabetic instruction and management
  • Fall Prevention and Safety
  • Pain management
  • Post-surgery care (wound care and dressing changes, ostomy care, amputation care)
  • Heart Failure program / Follow-up care
  • Wound care treatment with therapy for optimal improvement
  • Bowel and bladder management

Physical Therapy

Care by our trained Licensed Physical Therapist includes, but not limited to:

  • Assess prior level of function and complete evaluation to set up goals with patient
  • Environment assessment and adjustment to promote safety and fall prevention
  • Identify need of appropriate assistive devices and proper and safe use (e.g. wheelchair, walker, cane etc.)
  • Relief of joint and muscle pain
  • Restore muscle function and mobility
  • Post-operative rehabilitation
  • Improve range of motion to obtain maximum potential (e.g. gait transfers, bed transfer, ambulation)
  • Family and caregiver education with home exercise program and safety training

Occupation Therapy

Care by our trained Licensed Occupational Therapist includes, but not limited to:

  • Complete skilled evaluation to set up goals
  • Environment assessment and home adjustment to keep safe especially for bathing, grooming and dressing
  • Energy conservation techniques, especially if you get short of breath
  • Techniques to perform Activities of Daily Living safely
  • Assistive device training e.g. walker, cane, wheelchair
  • Patient, family and/or caregiver education

Speech Therapy

Care by our trained Licensed Speech Therapist includes, but not limited to:

  • Assistance with swallowing
  • Address brain injury and/or cognitive disorder
  • Slurred speech therapy
  • Promoting healthy lifestyle

Medical Social Work

Care by our dedicated Medical Social Workers includes, but not limited to:

  • Evaluation and assessment of patient and caregiver needs
  • Community resource coordination and mobilization  
  • Assistance with financial aid
  • Crisis intervention and counselling
  • Longer term care planning
  • Assistance with Advanced Directives
  • Patient and family education on your condition

Registered Dietician

Care by our dedicated Registered Dietician includes, but not limited to:

  • Develop and implement a nutritional plan based on an assessment of nutritional needs
  • Communicate with case manager regarding patient’s progress toward goals.
  • Provide education to the patient and family on appropriate diet.
  • Evaluate and instruct on nutritional principles, dietary plans, food selection and economics.

Home Health Aide Assistance

  • Provide and assist with personal care
  • Assist with ambulation
  • Provide and/or assist with bath and/or showers
  • Patient, family and/or caregiver education on providing and performing safe care
